When a preschool student is found eligible for and in need of special education services, he or she can receive those services through the Bainbridge Island School District.  Thus, he or she will be enrolled in the District on a part-time basis, even if the preschool student is receiving only therapy services (SLP, OT or PT).

Below are the forms needed for a part-time enrollment with BISD.  These forms help staff ensure each child’s safety and access to necessary contact information in case of emergencies, as well as increase awareness of any health considerations of the child.  Additionally, some of these forms are required by the State of Washington.                         

• Student Registration
• Certification of Immunization Status
• Student Health Information
• Home Language Survey
• McKinney-Vento Survey Form (if applicable)
• Emergency Student-Release Permission
• Emergency Contact/Health Information card for Elementary Schools
• Copy of the student’s birth certificate or passport to verify birthdate
• Copy of a document (such as electric bill) verifying student’s home address

These forms must be completed and returned PRIOR to the start of special education services. These forms should be filled out online.
